- Responsible for the visitation, assessment, intervention and multidisciplinary liaison relating to assigned in-patient areas within 24-hours of patient admission.
- Responsible for assessing the patients upon admission on the basis of NRS score, levels of existing or possible malnutrition and prioritizing high risk patients. Also responsible for patients referred by physician. The patient nutrition assessment is documented in Patient Medical Record File.
- Responsible to plan, develop & implement approved clinical nutrition SOP's -Policies/procedures/system/guidelines & system reports/monitoring tools. Responsible to develop disease specific nutrition care handouts in consultation with faculty chair.
- Responsible to undertake effective management of outpatient clinics (new/follow-up patients) when assigned & to ensure that work is completed timely and appropriate tools/systems are followed and records are maintained.
- Responsible to ensure that patient therapeutic/non-therapeutic diet is in accordance with physician diet order entry to comply patient individual nutritional assessment, patient likes and dislikes & patient food allergies are noted.
- Responsible to provide disease specific nutrition education/ guidelines/Diet plans to patients & families regarding nutritional care & appropriate Medical Nutrition Therapy.
- Making sure patients and their families follow up and review the diet plan according to ingredient availability and/or affordability.
